Beard Transplant Surgery

Before undergoing beard transplant surgery, it is essential to get detailed information from a medical consultant or doctor. The medical consultant should inform you about what to do and avoid before the surgery. In this conversation, any medications you regularly use, allergies, and any previous surgeries should be mentioned.

On the day of the surgery, the new beard line and the donor area are first determined with the doctor. Then, tests are conducted for contagious diseases, and if the results are negative, the donor area and beard area are shaved, and thus the surgery begins.

Beard transplant surgery typically takes 6-8 hours. The surgery is performed under local anesthesia. There are three types of anesthesia:

Needle anesthesia, needle-free anesthesia (dermojet), and sedation.

In needle-free anesthesia, the anesthetic is applied to the required area using a tool called a dermojet, which uses pressure. Since there may be minor bruising in the area after this procedure, it is applied only to the back of the head, and not to the beard area. Once the area is numbed, the process continues with needle anesthesia.

In sedation, a sedative is administered intravenously by the anesthesiologist. The patient remains semi-conscious while needle anesthesia is applied to the donor and recipient areas. Once fully numbed, the patient is awakened with medication. This way, the patient feels absolutely no pain during or after the surgery.

In normal needle anesthesia, the sensation of pain, discomfort, and burning lasts for about 5-10 minutes.

Beard transplant surgery can be performed using different techniques. These include FUE (Follicular Unit Extraction), Sapphire FUE, and DHI (Direct Hair Implantation) techniques. In the FUE and Sapphire FUE techniques, the surgery consists of three stages, while in the DHI technique, it consists of two stages.

In all three techniques, the first stage, which is the extraction process, is the same. Using a tool called a micro motor, grafts (hair roots and tissues) from the back of the head are extracted one by one and collected. The tissue size of each person varies (both in width and length). Therefore, it is important to extract the grafts without damaging the tissues. Otherwise, the transplanted hair will not hold and will fall out. This process is usually carried out by nurses. One nurse separates the grafts from the skin, while another nurse collects the separated grafts one by one.

The second stage is the process of creating small holes in the recipient area, which is called the channel opening process. A number of channels are created corresponding to the number of collected grafts. In the FUE technique, the holes are opened with a medical blade, while in the Sapphire FUE technique, the holes are opened using a pen with a sapphire tip. This is the main difference between the two techniques. This stage is performed by the doctor. The direction and density of the transplanted grafts depend on this stage.

In Sapphire FUE, the channels opened heal faster compared to FUE, and denser graft implantation is possible. However, not every surgery should be performed using the Sapphire FUE technique. For skin that has had previous surgery, scarred skin, or tougher skin, the FUE technique is more advantageous. After this stage, the patient is offered a meal, and then the final stage, the implantation process, begins.

In the implantation process, the collected grafts are implanted one by one into the channels that have been opened. This procedure is completed by two or three nurses working together to finish the surgery.

In the DHI technique, the channel opening and implantation processes are done simultaneously by the doctor. The nurses load the grafts into a tool called the Choi Pen one by one, and the doctor uses the pen to both create the hole and implant the grafts into the opened channel. The DHI technique emerged after FUE and is faster in terms of recovery compared to FUE. However, it is still up to the doctor to decide if this technique should be used. The DHI technique is more suitable for up to 4000 grafts and is generally applied in cases of unshaven transplants or when filling sparse areas of the beard.

At the end of the beard transplant surgery, only the donor area is bandaged. When the patient comes in for a check-up the next day, the bandage is removed, the check-up is completed, and the first wash is performed.

After Beard Transplant

The after beard transplant (post-beard transplant surgery) process is just as important as the beard transplant surgery itself.

Our medical consultants provide the patient with necessary post-surgery information through verbal, written, hands-on, and video demonstrations.

During the first wash after the beard transplant surgery, the patient receives detailed instructions either through hands-on demonstrations or via a video. For 10 (ten) days following the surgery, the patient continues washing their beard in this manner and sends daily photos of their hair and beard to our medical consultants for monitoring.

Five (5) days after the surgery, any swelling (edema) that developed is reduced, and the crusts formed in the donor area (the area on the back of the head where the grafts were taken) begin to shed.

By the 10th (tenth) day wash, all the crusts on the beard are shed. At this point, when the patient looks in the mirror, they will see all the transplanted beard hair.

The healing process of the redness varies from person to person. Afterward, some or all of the transplanted hairs may shed within the first month after the surgery. This is called shock shedding. The reason for shock shedding is that the beard transplant surgery is essentially a tissue transplant. It is normal for the transplanted tissue to shed the hair first as it adapts to the new area.

The hairs that shed due to shock shedding begin to grow back approximately 3 (three) months after the surgery. The rate of regrowth also varies from person to person. Pre-Surgery Instructions for Beard Transplant Surgery

  • To ensure the success of the beard transplant surgery, it is essential to avoid consuming alcohol, blood thinners, or similar substances for at least a week before the surgery. This is because thinning of the blood can result in a less dense transplant.

  • Physical exercise should be discontinued a week prior to the surgery as it speeds up blood circulation. If excessive bleeding occurs during the surgical incisions, the surgeon may not be able to see the proximity of the incisions, resulting in a less dense hair transplant.
  • Before the beard transplant surgery, hair and beard should not be trimmed. On the day of surgery, they will be shaved by healthcare professionals after the doctor’s consultation (do not shave the hair completely beforehand).
  • Starting the day before the surgery, the patient should limit tea to a maximum of one cup per day and avoid coffee or green tea entirely. After the surgery, if the patient wishes to consume these beverages, they should consult their medical advisor.
  • It is important to have breakfast on the morning of the surgery. However, if sedation anesthesia is used, it is strictly forbidden to eat or drink anything after midnight.
  • Lunch will be provided by the clinic/hospital during the surgery.
  • Since the surgery will last an average of 7 hours, it is advised not to wear heavy-scented perfume.
  • On the day of the surgery, patients should wear or bring loose, buttoned, or zippered clothing that can be easily removed or put on without rubbing against the treated areas.

After Beard Transplant

Post-Surgery Instructions for Beard Transplant Surgery

After the beard transplant surgery, follow these instructions:

  • Medications prescribed after the surgery should be used as directed. On the first night, there may be some leakage from the bandage applied to the back of the head, so place a towel on the pillow while lying on your back. It is normal to have some staining and discharge from the donor area on the back of the head (if there is bleeding, inform the medical consultant).
  • During the surgery, fluid is injected under the scalp to prevent damage to capillaries. This fluid is expelled from the body over approximately four days through sweat, urine, and the open pores of the donor area on the scalp. Therefore, it is important to rest the day after the procedure. Try to keep your head tilted back at a 45-degree angle and avoid bending forward. Drink about three liters of water daily. If this isn’t followed, the fluid may move from the forehead to the eyes. While this poses no health risk, it can be visually disturbing.
  • For the first ten days, avoid any impact or friction on the transplanted area (e.g., getting in and out of vehicles, changing clothes, lying down, etc.).
  • Use the neck pillow as instructed by the medical consultant to support the neck while lying on your back. Avoid sleeping on your side or face down. Ensure that the neck pillow does not rub against the transplanted beard area.
  • During the healing process and even in the future, avoid scratching any part of the head or beard (including areas that were not involved in the transplant or harvesting) with nails. For the first ten days, gently touch and release itchy areas with your fingertips without rubbing. After ten days, you can rub the itchy area in circular motions, like a massage, using your fingertips.
  • For the first ten days, do not wear a hat or any similar item.
  • Refrain from sexual activity for three days after the surgery.
  • Avoid alcohol and smoking for ten days after the surgery.
  • For fifteen days following the beard transplant surgery, avoid swimming, hammams, saunas, tanning beds, steam baths, and physical exercise.
  • The donor area on the scalp can be shaved with a trimmer 30 days after the surgery. For the next six months, avoid pressing the trimmer too hard on the skin.
  • The transplanted beard area can be trimmed with scissors one month after the surgery. After six months, it can be shaved with a trimmer. Do not use a razor or straight edge for one year, and if possible, avoid using them permanently.
  • Hair dyeing can be done four months after the surgery if using herbal dyes, and six months if using non-herbal dyes.
  • Numbness and itching in the treated areas may continue for up to six months.
  • One month after the beard transplant surgery, some or all of the transplanted hairs may shed, but they will start to regrow three months later. As hair growth continues for up to one year, you should send monthly photos to the medical consultant for follow-up.
