During this time, local anesthesia is applied to the entire scalp without the patient being aware. Once the scalp is completely numbed, the patient is awakened with another intravenous injection. This process takes just 15 minutes. Throughout the procedure, and afterward, the patient experiences no pain or discomfort.

After the anesthesia process is complete in a DHI hair transplant surgery, the first step, the extraction process, begins. Grafts are individually extracted from the donor area using the micro motor method and collected in a special solution container.

During the extraction process, the width and depth of the patient’s tissue are measured to ensure that all grafts are carefully removed without damaging the tissue. In the DHI technique, channel creation and implantation are performed simultaneously with a tool called the Choi pen. The tip of the Choi pen is both sharp and hollow. Nurses load a graft into the top of the Choi pen. The doctor then uses the pen to adjust the angle and density, making an incision in the scalp. The doctor presses the top of the pen to release the graft into the created channel. During surgery, an average of six Choi pens are used, with two or three nurses assisting. The nurses load each Choi pen with grafts, and the doctor implants them into the scalp.

• Medication: Follow the prescribed medication as instructed on the prescription.
• First Day Precautions: Since there may be leakage from the bandage in the back of the head, you should sleep on your back with a pad wrapped around the provided neck pillow to prevent any fluid or blood from staining the pillow (inform your medical consultant if bleeding occurs).
• Fluid Discharge: The fluid injected under the scalp during the surgery usually takes about four days to be expelled from the body through sweat, urine, and the open pores on the scalp. Therefore, rest after the procedure is essential. Keep your head tilted back at a 45-degree angle as much as possible to avoid bending forward and drink about 3 liters of water daily. If not managed properly, the fluid can move towards the forehead and eyes, which is not harmful but can be visually unpleasant.
• Use of Neck Pillow: Post-surgery, you should sleep on your back using the neck pillow as described by your medical consultant to support your neck. Avoid sleeping on your side or face down.
• Avoid Impact: Be cautious not to bump or rub the implanted area for the first ten days (be careful when getting in and out of vehicles, changing clothes, sleeping, etc.).
• Dietary Restrictions: Avoid salty foods for the first five days after the surgery.
• Itching: Do not scratch any part of the head (even areas where grafts were not placed) with nails. In the first 10 days, only gently touch the itchy area with the tips of your fingers without scraping. After 10 days, you may gently massage the itchy area using the palm or fingertips in a circular motion.
• Avoid Headwear: Do not wear hats or similar items for the first ten days.
• Abstinence from Sexual Activity: Refrain from sexual activity for three days post-surgery.

The transplanted area should be cut with scissors three months post-surgery, and razors or blades should not be used for at least one year, preferably never again.
• Hair Dyeing: If the dye is herbal, you can dye your hair four months after the surgery; if not herbal, wait six months.
• Numbness and Itching: You may experience regional or spot numbness and itching in the treated areas for up to six months.
• Hair Shedding and Regrowth: Some or all of the transplanted hair may shed one month post-transplant and begin to regrow three months later. The full regrowth process takes up to one year, so it’s important to send monthly photos to your medical consultant for monitoring.
